About us

At Pathway Support Network, we are a team of independent support workers who've come together with a shared passion for filling the gaps in care and providing more personalised, genuine support within the NDIS space. Based in Melbourne’s western suburbs, we proudly support participants throughout the surrounding areas with a flexible and person-centred approach tailored to individual needs and goals. With experience across a range of support areas, we have the capacity to provide compassionate, reliable care to participants with diverse backgrounds and support requirements.

Social engagement

  • Assistance to participate in social activities, clubs, and community events, fostering connections and reducing isolation.

Group activities

  • Guidance and support to engage in community programs such as group sessions, skill building classes, hobby activities and peer support groups. 

Access to local services

  • Support in accessing local amenities, transport, and essential services to promote community integration and health and well - being.

Volunteering opportunities

  • Connecting participants with volunteer roles that align with their interests and contribute to the community.

Peer support networks

  • Connecting individuals with peer groups for mutual support, shared experiences, and friendship.

Personal care support

  • Assistance with daily personal activities, tailored to individual needs and promoting self-sufficiency.

Travel & transport training

  • Guidance and practical support to build confidence and skills in using public transport or other travel options.

Household management

  • Support with household tasks, meal preparation, and organisational skills to maintain an independent living environment.

Employment pathways

  • Assistance with job seeking, resume building, interview skills, and workplace support for meaningful employment.

Administration 

Support in managing day to day administrative tasks such as budgeting, promoting routines, setting reminders and attending to paperwork to create healthy lasting habits

Collaborative care

Advocacy support

  • Assistance in understanding and advocating for NDIS rights and accessing relevant information.

Allied health coordination

  • Working closely with occupational therapists, physiotherapists, speech pathologists, and other allied health professionals.

Care team integration

  • Seamless communication and collaboration with existing care teams to ensure holistic and coordinated support.

Family involvement

  • Engaging families and guardians in the support process to ensure a comprehensive and shared understanding of goals and progress.

Goal planning & review

  • Ongoing assessment and review of individual goals, adapting support plans to meet evolving needs and aspirations.
